This study investigated uptake of advance care planning, including completing an advance care directive and appointing an enduring guardian, among Australian adults attending hospital outpatient clinics. Only 20% of participants had completed an advance care directive, and 35% had appointed an enduring guardian. Uptake of advance care planning may be improved with community education and training of community-based care providers in supporting advance care planning.
